DESCRIPTION:The 40th Ward\, once again\, be hosting the Back to School Event with the Chicago Department of Public Health. Immunizations will be administered to kids birth to 18 years of age. Please make sure to bring your child's immunization history with you. \n\n \nThe clinic provides all immunizations that are recommended by the CDC\, with the exception of varicella (chickenpox) vaccine.\n\nWe will also be offering school supplies to assist students in getting ready for the upcoming school year.
