• Apple Fest 2019 - Food Vendor

    Apple Fest takes place on Saturday, October 5 and Sunday, October 6 from 9am-6pm. The festival is located on Lincoln Avenue between Eastwood and Lawrence Avenues.

    Food Vendor Applications MUST be completely filled out and submitted by June 28, 2019.
    The only paperwork due by June 28 is this completed Apple food vendor application.
    Other due dates are:
    August 2 / Payment and all supporting paperwork (applications list all required items)
    August 23 / Final Food menu
    August 30 / Equipment rental application (if needed)

    Vendors filing late applicationa will be assessed a $50 late fee.

    The Lincoln Square Ravenswood Chamber of Commerce (LSRCC) and Big Buzz Idea Group (BBIG) are not responsible for applications that do not include the necessary documentation.

    Vendors with outstanding debt to the LSRCC or the City of Chicago will not be allowed to participate until outstanding financial obligations are rectified.

    This is a rain or shine event. Inclement weather or Acts of God may result in the cancellation of the event, but not the refunding of fees.

    Menu Items (All menus MUST have an apple or Fall-themed item and be pre-approved by Apple Fest Committee. Breakfast menu item STRONGLY recommended.)
    Will you provide your own tent and weights? *
    Tents/Tables are NOT provided with this application. Tents, Tables, Chairs, and other equipment can be rented from the festival at an additional charge. Weights are REQUIRED to be utilized with tents. Failure to comply with this will result in removal from the event.
    Will you need electricity? *
    Electricity is NOT provided with this application. Electricity and other equipment can be rented from the festival at an additional charge, and will require submitting a Equipment Rental Form.
    VENDOR FEES
    Please select the appropriate vendor fee and number of spaces needed. Food Vendors are also required to complete a Festival Application, DCASE Temporary Food Vendor License Application (with final menu), copy of current Health Inspection (dated 2019), Summer Sanitation Certification and Certification of Insurance verifying Commercial General Liability Insurance with the City of Chicago as the Certificate Holder and with the LSRCC and BBIG named as additional insureds. The LSRCC does not withhold any taxes from Apple Fest participants and any taxes associated with Apple Fest participation are the responsibility of each individual Food Vendor.

    Each additional space will be charged at the 10x15 space. Only one license fee is required for the City of Chicago.
    Are you a Non-Profit organization? *
    NON-PROFIT DISCOUNT: Non-profit organizations must provide verification of non-profit status and a copy of the organization’s 501(c)3. Food Vendor application fees are 15% off for all non-member organizations and 25% off for all member organizations.
    Please tally up the total of your application fees based on your vendor pricing and number of spaces needed. (e.g. LSRCC Member that needs two vending spaces: $600 x 2 = $1,200 + $25 City Fee = $1,275).

    Not all of the below items are required at time of application submission. Once your Apple Fest application is approved you will be contacted on how to submit the following documentation. However, if you do have any of the below information please upload with this application.

    City licensing and compliance with the Department of Health and Sanitation is the responsibility of each Food Vendor.
    VENDOR RULES AND GUIDELINES
    Submission of a Vendor Application does not guarantee participation in the event. All applicants are reviewed by the Event Committee and notified of their acceptance. Any vendor not approved for the event will be notified and will receive a full refund of application fees.
    Booth Requirements *
    • Vendors are responsible for setup and teardown of all signage, tents, tables and equipment. Tents must be weighted. You may use only your assigned space. No drilling into the street shall be permitted. • Vehicles are only allowed on the street for setup prior to the event and for teardown after the festival has closed. You will be notified when it is safe to bring your vehicle onto the street. All vendors will receive a confirmation email regarding load-in and load-out times. At no time can vendors block residential alleys or driveways. • All tents, booths, or canopies must be made from fire retardant material. Each Food Vendor must have an approved fire extinguisher (carbon dioxide or multi-purpose dry chemical—minimum of twenty pounds) in their booth. A type K extinguisher is required for grilling. • Food sales can be postponed until propane tanks are properly secured. All propane tanks must be 10 feet away from any canopy. The City of Chicago requires all propane tanks to be chained together and attached to a stationary post. • All Food Vendors are responsible for complying with all City of Chicago and State of Illinois guidelines for food service to the public. • In an effort to protect your space and effectively maintain cleanliness of the event, any vendor not staffing their booth for the duration of the event will be required to “close their booth” using tent sides. Vendors who do not adhere to this policy will be fined $100 per day. • Sidewalks are NOT part of assigned booth spaces. Sidewalks must be left unobstructed at ALL times. Any Food Vendor using any portion of the sidewalk will be fined $300 per day. • Each vendor is required to empty garbage, recyclables and grease as necessary in the Festival-provided containers. DO NOT dispose of grease in the street gutters. Protective materials must be placed under fryers and grills. Vendor space must be kept clean and free of debris. An event staff member will periodically monitor and review the space for cleanliness. Any booth not properly cleaned prior to exiting the festival will be fined $200. Any Food Vendor who leaves equipment or supplies behind at the close of the event will incur an additional $300 fine (NO exceptions). • Running water and lights are not provided. However, communal hand washing sinks are provided for Food Vendors. • Each canopy leg must be secured with at least 40 lbs. Please note that Apple Fest does not have extra weights on hand for vendor use. Canopies should be lowered and secured overnight.
    General Guidelines *
    • All event participants and their employees are expected to serve as a positive representation of the community and at no time should there be any illegal activities taking place at the event. Any vendor found in violation of these rules can be fined and immediately removed from the event and prosecuted at the Organizer’s discretion. • Mistreatment of event staff or volunteers will not be tolerated. BBIG/ LSRCC reserves the right to remove any individual or company from the event and ban from future Big Buzz Idea Group productions. LSRCC and BBIG employees, related festival providers and participating sponsors shall NOT be responsible for any injury, loss or damage that may occur to the vendor, its employees or property prior to, during or subsequent to the period covered by the vending contract. The vendor signing this contract expressly releases all of the aforementioned from any and all claims from such loss, damage or injury. • LSRCC is not liable for injury or litigation arising from the products/ services you or your company offer to the public. Products offered must comply with applicable state and federal laws. • This event will be photographed and/or videotaped. By participating in this event you hereby consent to the use of your likeness and/or image in those photographs and/or video for future promotional consideration by Apple Fest, the Lincoln Square Ravenswood Chamber of Commerce and/or Big Buzz Idea Group.

    Checks should be made to the Lincoln Square Ravenswood Chamber of Commerce. MasterCard, Visa, Discover, and American Express are accepted. Payments MUST be received by August 16, 2019 in order to not receive a late fee of $50.
